Building Your Community Resilience Map – A Personal Blueprint

Building Your Community Resilience Map – A Personal Blueprint

By Primary Health Awareness Trust | Social Connection & Community Wellbeing Series

Why Map Your Social World?

We often know when we feel lonely — but we don’t always know why.

Creating a Community Resilience Map helps you reflect on your current connections, identify what’s missing, and take small steps to strengthen the support around you.

This is not about pressure. It’s about perspective.

How to Use the Map

There are five types of connection that protect wellbeing:

  1. Emotional – Someone you can talk to about feelings or fears
  2. Practical – Help with shopping, lifts, or daily needs
  3. Community – Local groups, faith settings, or neighbours
  4. Creative – Places where you express, share, or grow
  5. Digital – Tech that connects you to family, services or support

In each category, write down:

  • ✔ Who supports you
  • ✉️ Who you’d like to reconnect with
  • 🌱 What you’re curious to try

Printable Worksheet Available

To help you begin, we’ve created a printable version of the Resilience Map. You can fill it out by hand or with someone you trust.

There are no right or wrong answers — just your truth. It’s a living document, and it can grow as you do.

🧠 Surprise Cognitive Prompt:

“Design your ideal weekly rhythm of connection — include solitude, friendship, community, creativity and nature.”

Mapping your ideal week stimulates the brain’s planning and goal-setting networks — encouraging personal agency and emotional resilience, especially during periods of change or isolation.

What Happens When You Map Your World

Many people feel relief when they see their social world laid out clearly — even if it’s sparse. Because now they have something to build from. Something real.

And the act of mapping itself is an act of care. It says: *I matter. My wellbeing matters. And I can make change, gently, in my own time.*
